CODE PERMITTING

Please correct the field(s) marked in red below:

For Pyrotechnic/Firework displays complete the following:

Provide a complete list of the types and sizes of devices to be used (all relevant details indicating full compliance with requisite code, standard and referenced requirements)
Provide a detailed site plan indicating required separation and fallout distances.
Provide proof of $1,000,000 general liability insurance listing the “Town of Chapel Hill Fire Department” as additionally insured
Provide a copy of any and all training certifications for the shooters to conduct the shoot.
Provide a list of names and ages of each person working with the on-site operation.

For Fire Sprinkler/Standpipe complete the following:
Attach 1 electronic copy of plans saved in PDF and CAD format (include calculations and cut sheets)
Attach 1 copy of hydraulic calculations with minimum safety margin of 10 psi.
Attach product cut sheets.
Attach letter of design certification indicating that system design as submitted meets all local and state requirements and standards set forth by NFPA.
Attach completed permit application:

For Fire Pumps, Pump Controllers and Related Equipment complete the following:
Attach 1 electronic copy of plans saved in PDF and CAD format (include calculations, data and cut sheets, type of pump rating, etc.….)
Attach 1 copy of hydraulic calculations, pump curve data, etc.….
Attach product/manufacturers data, cut sheets
Attach letter of design certification indicating that system design as submitted meets all local and state requirements and standards set forth by NFPA.
Attach completed permit application:

For Automatic Fire Suppression (Hood Suppression/ Alternative Systems) complete the following:
Attach 1 electronic copy of plans saved in PDF and CAD format
Attach a copy of nozzle selection criteria
Attach completed permit application:

For Blasting complete the following:
Explain the blasting schedule and detailed site plan (all relevant details indicating full compliance with requisite code, standard and referenced requirements)
Provide information on type and quantity of explosive to be used
Attach copy of licensing as an explosive blasting operator
Provide proof of $1,000,000 general liability insurance listing the “Town of Chapel Hill Fire Department” as additionally insured
Provide verification that blast mats and seismic monitoring are utilized on every blast/shot.
Attach completed permit application:

For Tents/Membrane/Air Supported Structures complete the following:
Provide site plan indicating location(s) of proposed tent on site (all relevant details indicating full compliance with requisite code requirements).
Provide detailed plan of proposed use and internal configuration of tent (tables, chairs, exits, etc.…)
Provide a copy of Flame Retardant Certificate
What is the proposed occupancy (i.e. the anticipated number of people inside the tent)?

For Spray/Dipping Operations complete the following:
Provide site/facility plan (all relevant details indicating full compliance with requisite code, standard and referenced requirements).
Provide booth detail 
Provide a copy of Building Permit

For Fire Alarm System complete the following:
Attach 1 electronic copy of plans saved in PDF and CAD format (include calculations, data and cut sheets, type of pump rating, etc.….)
Provide 1 copy of battery calculations
Attach device cut sheets.
Attach letter of design certification indicating that the system design as submitted meets all local and state requirements and standards set forth by NFPA 72 (2013 edition as of January 1st, 2016)*
Attach completed permit application:
In accordance with Town of Chapel Hill Accidental Alarm Program requirements, all fire alarm systems shall be registered within 10 days of approved installation – registration can be completed through the below links:

For Flammable/Combustible Liquid Facilities (Tanks / Piping / Installation / Removal) complete the following:
Attach site plan showing locations of all tanks/piping/vents (all relevant details indicating full compliance with requisite code requirements such as distances to buildings, lot lines, roadways, etc...)
Provide a copy of Zoning Compliance Permit or letter from Planning Division.
Provide tank details
Provide removal plan
